Patient Reminders

PCP referral: Please review your insurance policy requirements to determine if you need a referral from your primary care physician prior to your visit. Call before you arrive to confirm that all needed referrals have been received.

Insurance information: Please remember to bring your insurance card to your appointment. Also, alert the office if your insurance information has changed and needs to be updated. You can call the office to confirm that Dr. Wolf is a provider under your coverage. Your insurance may require a co-pay for your visit, and this should be paid at the time of your appointment.

New appointments: In order to expedite your check-in and minimize waiting times, we ask that new patients arrive at the office 15 minutes prior to their appointment. Please fill out the appropriate "appointment forms" below and bring them with you on the day of your appointment.

Appointment cancellations: We request that appointments be cancelled at least 24 hours in advance as a courtesy to Dr. Wolf and the other patients. When an emergency or illness arises, we will do our best to reschedule your visit as promptly as possible.

Clothing: Please wear or bring appropriate clothing to your visit in order to allow good exposure of the part being examined. Gowns will of course be provided for you at the time of appointment if necessary.

Additional material: Please bring all pertinent X-rays, MRI's, CT Scan's, and EMG reports to your appointment. Also please have any relevant office notes and previous operative reports with pictures forwarded from your previous physician's office. This information will allow Dr. Wolf to best treat you. Also please bring a list of your medications and the doses of each for your record.

Appointments

Appointments can be made by calling The Wolf Orthopaedic Center between the hours of 8:30 a.m. and 5:00 p.m. Monday through Friday and asking for an appointment with Dr. Wolf. After hours, a message can be left with the answering service and your call will be returned during the next business day.

Please read the "Appointment Info" section for further details which will facilitate your visit and answer many questions. Please also fill out the appropriate "Appointment Forms" below if you are a new patient or have a new problem.

Waiting times: We strive to minimize any wait on the part of our patients. Occasionally emergencies do arise, and you will be informed promptly of any delay in your appointment. We ask that our patients contribute to efficient patient flow by being on time for their appointments and by bringing all requested material with them.

Prescription Refill Policy

Routine medication refills will only be given during business hours, Monday through Friday, 8:30 a.m. to 5:00 p.m. Refills after hours and on weekends are done only for emergencies. This policy is designed to prevent medical errors and to allow for proper documentation of prescriptions.

Patient Forms

Below are several forms which will help Dr. Wolf best care for you and treat your problem. New patients should fill out the Demographics Form, Health History Form, and the Injury Form related to your presented complaint. They should be printed and brought to your appointment. The same forms will also be available in the office for your convenience. Established patients with a new complaint only need to fill out the appropriate Injury Form.

Insurance Forms

Our office will complete health insurance claim forms on the patient's behalf free of charge. If you are covered by an insurance company that requires pre-certification and authorizations for procedures or other services, please inform our office so that we can assist you.
